During recent years, the diagnosis of acute coronary occlusion has been greatly facilitated, for example, by the widespread use of serum and urine enzyme determinations. More definite means of direct and d i e diagnasis makes for early recognition of complications and for the avoidance of therapy that may be contraindicated. Serious complications of acute coronary occlusion include shock, very slow or very fast ventricular rates, advanced heart failure, embolic events, the postmyocardial infarction syndrome and rupture of either the ventricular wall, interventricular s e p tum, chordae tendinae, or papillary muscles. Correction of severe hypotension in shock may be accomplished either by increasing the peripheral vascular resistance or by increasing cardiac output. The sympathomimetic drugs commonly employed increase peripheral vascular mistance. Newer experimental approaches include the use of hypothennia, extracorporeal circulation, or mechanical augmentation of the diastolic pressure. Glucose induces insulin secretion via metabolism in order to increase ATP, closing the KATP channel, leading to increases in the intracellular calcium concentration. ATP, and perhaps other glucose metabolites, also has direct effect on insulin exocytosis. Zaitsev's group has studied organic compounds possessing activity similar to GLP-1, which can be administered orally. Phentolamine, an -adrenergic blocker with an imidazoline ring, stimulates insulin secretion. Another insulinotropic imidazoline, RX871024, a close analog without adrenergic blocking effect, increases islet insulin secretion. In vitro, it inhibits the KATP channel, and in vitro, it increases islet insulin secretion in a glucose-dependent manner. It also has a direct KATP channelindependent ; effect in increasing insulin secretion at high- but not low-glucose levels, but only in diazoxide-treated depolarized cells and in "permeabliized" islet cells with membrane defects. The KATP-independent effect depends on protein kinases A and C. RX871024 stimulates increase in diacylglycerol levels in a fashion additive to that of the cholinergic agonist carbachol. A new generation of imidazolines with pure glucose-dependent insulinotropic activity includes BL11282. This potentiates glucose-induced insulin secretion without effect at low-glucose concentration, not acting on the KATP channel, with effect seen both under depolarized and permeabilized conditions, in KATP-deficient mice, and acting on protein kinases A and C. Zaitsev concluded that the new generation of imidazolines potentiate glucoseinduced insulin secretion and promote insulin exocytosis independently from blocking KATP channel, directly stimulat.

The GridCoord project aims at strengthening the co-operations amongst the European funding authorities and Grid projects in order to improve the coordination of the future activities in the field of Grid computing. The other aspect, that of "harm reduction proper", is about health care, syringe distribution, substitution programmes and so on.
The main side effects are headache, nausea and nervousness. Usually, these are self-limiting and can be helped, in some cases, by reducing the dose. In multiple sclerosis patients experiencing such side effects, it may be prudent to use half a tablet of the lowest dose 100mg ; for a while until symptoms disappear, then increase to a more appropriate dosage and prolixin.
