No one who has ever been within a hundred yards of a typical medical practice should have expected that physicians and patients would consistently follow the demanding monthly blood test schedule Parke-Davis suggested. My research unit had taken a look at how well physicians monitored patients prescribed tacrine, a weakly effective drug for patients with Alzheimer's disease. It was also manufactured by ParkeDavis, and also caused liver damage. There, too, the company had told doctors that the damage could be prevented by frequent blood tests to measure liver function. We scanned the records of several thousand patients prescribed tacrine and found that only a tiny fraction were actually getting the blood tests as the company recommended. Later on Dr. David Graham, an FDA epidemiologist, analyzed data from a large HMO to see whether patients given Rezulin in typical practice were actually being monitored as required. He found that by the third month of use, fewer than 5 percent of them were getting all the recommended blood tests. This "monitoring defense" reflected the supine posture often adopted by both drug manufacturers and the FDA throughout the 1990s: "We told you there was a risk, we put it in the official label, so it's not our fault if something bad happened." Near the end of 1998, another patient enrolled in a different company-sponsored Rezulin study developed rapid-onset liver failure despite regular monitoring; within a month she was dead. But over 2, 600 other patients throughout the country were still participating in that study. Parke-Davis had paid physicians a fee of up to 0 for each patient they enrolled. One Parke-Davis scientist asked in a memo, "Do we have to send a letter out to all of the REACH investigators to inform them of this event?" No, responded her colleague at the company, using capital letters to underscore the point. "We have NO REGULATORY OBLIGATION to send a letter to the [study] physicians . send out a letter now would be misleading because we cannot fully explain the case and it would be unnecessarily frightening." The company blamed the death on other causes, and continued to claim that the drug was safe as long as liver function was monitored. In the industry this is sometimes known as "defending the molecule." In March 1999, over a year after troglitazone had been withdrawn in Europe, the British Medicines Control Agency took a second look at the drug and ruled that it should remain off the market because the evidence showed that it could not be used safely. Most haemodialysis HD ; patients treated with recombinant human erythropoietin receive concurrent iron therapy. Although both oral and parenteral iron supplementation have been used in patients with stage V renal disease, the parenteral route has been found to be more effective. Polynuclear ferric hydroxide carbohydrate complexes, such as iron dextran, iron sucrose and iron gluconate, have been used widely; nonetheless, lately, iron sucrose was considered to be one of the safest agents [1]. Iron overload and intravenous IV ; infusion of iron have been incriminated in the increased susceptibility for infection [25] and the accelerated rate of atherosclerosis [6, 7], which are current complications in dialysis patients. Leucocytes play a key role in the defence against infection and are important mediators in the progression of atherosclerosis. Their function relies at least in part on the expression of surface molecules such as CD11b, CD14, CD36 and CD45, and on the generation of mediators such as the reactive oxygen species ROS ; . CD11b is mainly found on the surfaces of monocytes and granulocytes; CD14 is predominantly expressed on monocytes and macrophages. The surface molecule, CD36, belonging to the class B family of scavenger receptors, is expressed on monocytes, platelets, endothelial cells and on some lines of human tumour cells, but not on lymphocytes or granulocytes. Another leucocyte surface molecule, CD45, is abundantly found on granulocytes, lymphocytes, monocytes, eosinophils and basophils. Direction Tuning. Fig. 1A shows a front view of a set of 683 hand movements made by the monkey over 15 min. As described in greater detail in the supporting information, which is published on the PNAS web site, movements were individuated on the basis of the distinctive rising and falling speed profile of the hand. The firing rate is proportional to the cosine of the angle between the actual hand direction and the preferred hand direction. Fig. 1B shows the cosine fit to the data for this example neuron. The R2 value is 0.03, indicating that the direction tuning model accounted for almost none of the variance in neuronal activity. Sixty-three neurons were tested in this fashion. The black bars in Fig. 2A show the distribution of R2 values for all neurons. Almost all values were 0.1. The direction tuning model explained almost none of the variance in the sample of neurons. These results do not show that neurons in motor cortex are not direction-tuned. Rather, they show that, during free movement, when many movement parameters presumably contribute to the activity of neurons, direction tuning makes a vanishingly small contribution. Neurons in motor cortex have a consistent preferred direction when tested on a center-out task 1, 2 ; . Changing the start position of the hand or the start posture of the arm, however, can alter the preferred direction 35 ; . In our study, the lack of a clear preferred direction might have resulted from our use of a diverse movement set including a variety of start positions.
