ABSTRACT: The present study was conducted to assess a possible in vivo effect of propiverine, an anticholinergic drug to treat urinary incontinence and related disorders, on the activity of intestinal CYP3A4 and of hepatic CYP3A4, CYP2C9, CYP2C19, and CYP1A2. The activity of the respective cytochromes P450 was measured using the following metrics of selective substrates given as a tailored low-dose phenotyping cocktail: intestinal availability of midazolam 2 mg orally ; , clearance of midazolam 1 mg i.v. ; , apparent clearance of tolbutamide 125 mg orally ; , urinary excretion of 4 -hydroxymephenytoin 0 to 8 h postdose 50 mg of mephenytoin orally ; , and the paraxanthine caffeine plasma ratio 6 h postdose 150 mg of caffeine orally ; . These metrics were determined in 16 healthy young men at the end of 7 days of treatment with 15 mg of propiverine test ; or placebo reference ; twice daily. All phenotyping drugs were quantified by liquid chromatography-tandem mass spectrometry. Chronic propiverine treatment reduced hepatic and intestinal CYP3A4 activity slightly to 0.89-fold and 0.80-fold, respectively [90% confidence interval CI ; for test reference ratios 0.850.93 and 0.720.89], with the combined effect resulting in a 1.46-fold increase in area under the curve of oral midazolam 90% CI 1.361.57 ; . Propiverine had no relevant effect on CYP2C9, CYP2C19, and CYP1A2 90% CI for test reference ratios 0.931.00, 0.840.96, and 0.971.07, respectively ; . All study drugs were well tolerated. In conclusion, propiverine has a minor potential to cause drug-drug interactions. May also be at an increased risk of cognitive adverse effects associated with AED use such as aggression, hyperactivity, memory impairment, and impairment of learning ability. Specific concerns exist among elderly patients with seizure disorders as well. The elderly may be at an increased risk of developing adverse cognitive effects with AED use. In addition, the elderly are often taking multiple medications for various disease states, so drug-drug or drug-disease interactions are more of a concern. Plus, the elderly generally have reduced renal and hepatic function, which can result in the modification of drug dosing regimens in this population. MONITORING Drug concentrations The goal of therapeutic drug monitoring in the epilepsy arena is to optimize outcomes through the appropriate use of serum concentrations. Although all AEDs have suggested therapeutic serum concentration ranges, no therapeutic range applies to all patients across the board. Some will respond to concentrations below the lower limit of a therapeutic range, while others will require concentrations above the range. Indeed, the optimum serum concentration for an individual patient may depend on a variety of factors including patient response to the AED and severity of the seizure disorder. Therapeutic drug monitoring should not replace clinical judgment when evaluating a patient's response to therapy. DRUG INTERACTIONS Numerous drug-drug interactions exist both among the AEDs and between AEDs and other medications. A summary of various interactions between selected AEDs and other medications is presented in table 2.

The fead model failed, however, to unequivocally predict the therapeutic efficacy of mephenytoin or acetazolamide and meprobamate. As the intake of CAM is increasing, herbdrug interactions within oncology are likely to occur. In particular, lower therapeutic efficacy resulting from induction of metabolizing enzymes and drug transporters may not be easily recognized. It is therefore important that treating physicians are aware of the possibility of CAMdrug interactions. Unfortunately, the effects of CAMs on induction of metabolizing enzymes and drug transporters are generally not known and poorly studied. Some investigators have used probe substrates to explore the effects of herbs on the activity of specific CYP enzymes in healthy volunteers, like Gurley et al. [77]. Suitable substrates are caffeine for CYP1A2, tolbutamide for CYP2C9, mephenytoin for CYP2C19, dextrometorphan or debrisoquin for CYP2D6, and midazolam or erythromycin for CYP3A4. In addition, a cocktail of probe drugs can be used to explore the activities of multiple CYP enzymes [19]. From these type of studies, the physician already could derive specific recommendations for the use of various herbs in combination with chemotherapeutic treatment. However, phenotyping studies do have some limitations, such as marked intrasubject and intersubject variability and the possibility of interaction between coadministered probes [19]. Another disadvantage is the fact that, in most of these studies, herbs were used for a relatively short period of time, while for induction, in particular, chronic use is important.
