Recombinant human granulocyte colony-stimulating factor rhG-CSF ; is widely used to promote granulocyte recovery from a variety of pathologic states. Recombinant human interleukin-11 rhIL-11 ; has recently become available clinically as a platelet restorative agent after myelosuppressive chemotherapy. Preclinical data has shown that rhIL-11 limits mucosal injury after chemotherapy and attenuates the proinflammatory cytokine response. The potential efficacy of combination therapy with recombinant human forms of rhIL-11 and rhG-CSF was studied in a neutropenic rat model of Pseudomonas aeruginosa sepsis. At the onset of neutropenia, animals were randomly assigned to receive either rhG-CSF at a dose of 200 g kg subcutaneously every 24 hours for 7 days; rhIL-11 at 200 g kg subcutaneously every 24 hours for 7 days; the combination of both rhG-CSF and rhIL-11; or saline control. Animals were orally colonized with Pseudomonas aeruginosa 12.4.4 and then given a myelosuppressive dose of cyclophosphamide. rhG-CSF resulted in a slight increase in absolute neutrophil counts ANC ; , but did not provide a survival advantage 0 of 12, 0% survival ; compared with the placebo group 1 of 12 , 8% survival ; . rhIL-11 was partially protective 4 of 10, 40% survival the combination of rhG-CSF and rhIL-11 resulted in a survival rate of 80% 16 of 20; P F .001 ; . rhIL-11 alone or in combination with rhG-CSF resulted in preservation of gastrointestinal mucosal integrity P F .001 ; , lower circulating endotoxin levels P F .01 ; , and reduced quantitative levels of P. aeruginosa in quantitative organ cultures. Normal human tissue samples of general organs were obtained from three autopsy cases and from non-tumour parts of surgically resected specimens Table 1 ; . For human neoplastic tissue samples, 123 cases of surgically resected tumours including 21 germ cell tumours of ovarian and testicular origin were examined in the Department of Pathology and Cell Biology at University of Occupational and Environmental Health in Kitakyushu, Japan Tables 2 and 3 ; . These cases were classified according to the World Health Organization Histological Typing of each tissue. Hormonal therapy has been a cornerstone in the adjuvant treatment of surgically resected breast cancer that has measurable estrogen and or progesterone receptors ER and or PR ; . The advent of the aromatase inhibitors has resulted in efforts to incorporate these new agents in the adjuvant therapy of ER and or PR positive breast cancer in the post-menopausal population. Aromatase inhibitors, by reducing circulating endogenous estrogen levels by 95-98% in post-menopausal women, deprive the ER and or PR positive breast cancer cell of its major growth factor estrogen ; . They have all been compared to megestrol acetate for the second-line.

It has often been assumed that fluctuations of scalars with small molecular diffusivities Prandtl number P r 1 ; can survive at length scales smaller than the Kolmogorov scale. However, we demonstrate here that it would not be true to buoyant active scalars such as heat and salt, whose distribution affects the fluid motion. Results of direct numerical simulations for a double-diffusive system, i.e., a system with two active scalars which constitute density stratification, show that the fluctuations of active scalars with high Prandtl number disappear at the Kolmogorov scale. This implies that the fluctuations of heat P r 6 ; and salt P r 600 ; in the ocean, would disappear at much larger scales than have been considered. This suggests also the possibility of realizing numerical simulations with much coarser computational grids than have ever been thought. Similar results would be applied to other active scalars controlled by external restoring forces, such as electric or magnetic forces and memantine.
Must be added that results have never been validated with systematic analyses. There have also been numerous reports on the use of high-dose progestins HDPs ; , megestrol acetate MA ; , and medroxyprogesterone acetate MPA ; , for the palliative treatment of CACS. MA and MPA are both synthetic progestins derived from 17oc-hydroxyprogesterone, which are used in the treatment of hormone-dependent tumors. It was during the evaluation of antitumor activity in these cancers that the effect of both MA [24] and MPA [25] on increase of weight and appetite was discovered, independently of objective response. Subsequent studies showed that the anticachectic effect of HDPs could be mediated by a reduction in the production of cachexing cytokines [26]. It has also been reported that weight gain is not due to water retention but to an increase in total body fat and, to some extent, increased lean body mass [27]. To date, phase II studies, randomised studies and some traditional reviews have appeared in the literature dealing with the effect of HDPs on CACS. As far as we know, however, no one has yet carried out a systematic review based on current available data aimed at producing the most accurate and correct quantification of this effect. This article reviews the prospective randomised clinical trials RCTs ; identified through a literature search that evaluate the use of progestins in cancer cachexia syndrome. From the results of these studies major endpoints of clinical importance in CACS are pooled: weight gain and increased appetite.

MBTOC met 11-15 April 2005 in Buenos Aires, Argentina and for a second time in Melbourne, Australia, from 29 August to 2 September, 2005. These meetings were held as required by the time schedule for considerations of CUNs given in Annex I referred to in Decision XVI 4. MBTOC made an initial assessment of the 62 new or additional critical use nominations for 2006 and 27 nominations for 2007, totalling 324.68 and 8088.32 metric tonnes respectively. Of the 89 new or supplementary CUNs considered during its April 2005 meeting in Buenos Aires, MBTOC was unable to assess 26 nominations. Some of the larger and more complex nominations were in this category. It did not recommend two nominations. MBTOC submitted questions relating to those nominations classified as `unable to assess' in Buenos Aires that would assist their further evaluation during its September 2005 meeting in Melbourne. Two Parties made arrangements to meet with MBTOC during the Melbourne meeting for discussions with regard to their outstanding CUNs, in accordance with paragraph 8 of Annex 1 referred to in Decision XVI 4. In paragraph 20 of Annex 1 referred to in Decision XVI 4, Parties, inter alia, specifically requested that MBTOC explicitly state the central basis for the Party's economic argument relating to CUNs. Table 4 provides this information for each CUN. MBTOC has sometimes suggested quantities of MB for 2006 or 2007 different from that nominated. Grounds used for these changes are given in detail after the relevant CUNs in Table 4. The adjustments follow the standard presumptions given in Tables 1 and 2, unless indicated otherwise. In general, CUNs resulted mainly from the following issues: regulatory restrictions on alternatives, scale up of alternatives, and economic issues. For the most part technical alternatives exist, but often at a less developed state than methyl bromide. MBTOC has been unable to identify alternatives, or has inadequate information for the following applications: fresh high-moisture dates, some seeds when rapid turn around is required for immediate planting, cheese stores, dry cure ham treatment, and unmovable historical artefacts especially where fungi are of concern. The Parties are requested to consider focusing some research on these applications to identify and, where required, register effective alternatives. Further to the normal Disclosure of Interest required under the TEAP TOC terms of reference, MBTOC members made an additional disclosure relating specifically to their level of national, regional or enterprise involvement in the CUN process. This was required to ensure that those with a high level of involvement and interest in developing a particular nomination did not bias the process of evaluation through participation in the and meprobamate.

Lapatinib Helps Patients with EGFR-Expressing Advanced Kidney Cancer Live Longer A phase III clinical trial has shown that the investigational drug lapatinib slowed cancer growth and improved survival in a subset of patients with an advanced form of kidney cancer called renal cell carcinoma RCC ; that continued to grow despite standard therapy. The patients in the study who responded favorably to this drug had tumors that produced the greatest amounts of epidermal growth factor receptor EGFR ; , a protein that plays a key role in cell growth and division. "Although additional clinical trials are necessary, our findings suggest that lapatinib could be a new treatment option, along with immunological therapies and anti-angiogenesis agents, for patients with advanced renal cell carcinoma that produces high levels of EGFR, " said lead author Alain Ravaud, MD, PhD, Professor of Medical Oncology at University Hospital of Bordeaux and the University Victor Sgalen in Bordeaux, France. Although interleukin-2 and interferon-alpha are the standard treatments for patients with advanced RCC, only about 15% of patients respond to these immunotherapies. Sunitinib and sorafenib Nexavar ; are two new oral therapies recently approved by the FDA in the United States for treating RCC. Both target enzymes involved in angiogenesis the development of blood vessels that feed tumors ; . Lapatinib, which is also taken orally, inhibits two tyrosine kinase enzymes that drive cancer cell growth. These enzymes are part of the EGFR pathway, which is responsible for tumor proliferation and growth, and is found in many types of cancer. In this study, lapatinib was compared with hormonal therapy megestrol acetate or tamoxifen ; , which was considered a standard treatment when the trial was designed for patients with RCC that persists despite immunotherapy. Starting in December 2002, 207 patients with advanced renal cell carcinoma that continued to grow despite treatment with immunotherapy were randomly assigned to receive hormonal therapy, and 209 patients were given 1, 250 mg of lapatinib daily for a median duration of 12 weeks. When the group was analyzed as a whole through September 2005, overall survival and the time it took for the cancer to grow time to progression ; did not differ significantly between the two treatment groups. Since lapatinib targets the EGFR pathway, the trial was designed to assess patients according to the levels of EGFR produced by their tumors. Among 241 patients whose tumors produced the highest amounts of EGFR, time to progression was longer in the lapatinib group 15.1 weeks vs. 10.9 weeks for the hormonal therapy group ; , as was overall survival 46 weeks vs. 37.9 weeks ; . Lapatinib was generally well tolerated; common side effects included rash observed in 44% of patients vs. 3% of those on hormonal therapy ; and diarrhea 40% vs. 3%; usually minor or moderate ; .
