We have been asked numerous times how best to switch patients from Cardura XL to immediate release standard ; doxazosin. Cardura XL is currently in short supply and generic doxazosin is a suitable, and cheaper, alternative. Before switching, consider the appropriateness of the doxazosin prescription. The ALLHAT study showed doxazosin to be inferior to thiazide diuretics for hypertension and both NICE and BHS guidelines consider the use of alpha-blockers to be fourth-line therapy. Once-daily dosing is suitable for both XL and standard preparations as the half-life of doxazosin is 22 hours. However, the formulations differ in their bioavailability, with Cardura XL having around one half to one third the bioavailability of standard preparations. Therefore, there is a risk of hypotension when switching to the same dose of standard doxazosin. Options for switching are: ! to same dose e.g. 4mg XL to 4mg standard ; ! to half dose e.g. 4mg XL to 2mg standard ; For patients at risk of hypotension it is most appropriate to switch to half dose, and increase if necessary. If switching to the same dose, bear in mind that the dose may need to be decreased. Whichever dose is chosen, it is important to monitor blood pressure closely and adjust the dose if necessary. During Emerson's tenure as the first director of the Institute of Public Health at Columbia's College of Physicians and Surgeons from 1922 to 1939, his notundistinguished epidemiology faculty Alton Pope, Durward Jones, and later Edward Godfrey and Albert Hardy ; were graced with the title of Assistant Professor. Among leading medical schools of the time, this title was perhaps less lowly than it appears today. Only in 1940 were Ernest Stebbins in epidemiology and John Fertig in biostatistics appointed the first full professors in the School of Public Health. Stebbins went on to a distinguished career as Commissioner of Health for New York City and as Dean of the Johns Hopkins School of Hygiene and Public Health. E. Gurney Clark succeeded Stebbins in 1947, having done important studies at Hopkins on the natural history of syphilis. Like Emerson, he too was an epidemiologist of broad gauge. Clark was perhaps best known for his pioneering text, written with Hugh Leavell, on preventive medicine 4 ; , with its wellstructured guide to epidemiologic investigation and its still-famous classification of primary, secondary, and tertiary prevention. Clark's most imaginative endeavor was his studies, more or less contemporary with those of George Pickering at Oxford, of blood pressure in the population at large. He aimed to carry out the first full-scale examination of essential hypertension and its social, environmental, and personal antecedents 5 ; . For this purpose, he enlisted the help of the Bureau of Applied Social Research in the University, famously the breeding ground for many of the best research sociologists of the time. Clark himself was laid low by malignant hypertension, and in the later 1950s his faculties began to fail. Several papers were published during that period 6-11 ; , however--among them, a then-unique statistical application borrowed from the social sciences for testing the reliability and variability of blood pressure measures 7-9 ; . In 1966, the search committee for Clark's successor chaired by Ray Trussell, with Ernest Gruenberg and Jack Elinson ; was looking to continue an epidemiology program of broad social scope. It settled on me, then visiting from Manchester University; and, together with Zena Stein, I was persuaded. The committee was assuredly influenced by my first book, coauthored with William Watson, on sociology in medicine 12 ; . ; I occupied the chair from 1966 through 1978, when I assumed the founding Gertrude H. Sergievsky chair of the Sergievsky Center for Neurodevelopmental Epidemiology. This requires integration of different points of view in order to get an overview of both costs and activities, and sometimes makes it hard to trace the reasons behind particular choices in order to obtain a clear overall picture of efficiency. In any case, any efficiency assessment should be based on appropriate comparisons or benchmarks. This is a problem since suitable reference points are often difficult to identify. Activities within a project are often unique to the country and time. There is thus no reference point in a country and comparisons with other countries are not always appropriate. We tried to overcome this problem by assessing the efficiency of projects as a team exercise so that expertise over different countries and different types of projects was discussed at the same time to provide consistency across assessment. Germany ESCAT ; 149 showed that among 305 self-managed patients, INR values were more frequently in range 78% ; compared with 61% in 295 patients assigned to usual care. The rate of major adverse events was significantly different between groups: 2.9% per patient-year of therapy in the self-managed group versus 4.7% in the usual-care group P 0.042 ; . When patient self-management is compared with the outcomes of high-quality anticoagulation management delivered by an anticoagulation clinic, the differences between the 2 methods of management are less marked. Watzke et al150 compared weekly INR patient self-management in 49 patients with management by an anticoagulation clinic in 53 patients. There was no significant difference for time in therapeutic range between groups, but the self-management group had a significantly smaller mean deviation from their target INR. Cromheecke et al151 conducted a randomized crossover study with 50 patients managed by an anticoagulation clinic or by self-management. Although the differences did not achieve statistical significance, there was a trend toward greater time in therapeutic range in the self-management group 55% versus 49% ; . Preliminary results from 2 recent studies further suggest that when compared with anticoagulation clinic management, patient self-testing or patient self-management offers limited advantages. Stimulation techniques can improve the reliability of MEP recording further. These ndings have led to the need for signicant modications to the anaesthetic technique when corticalevoked MEPs are measured. In the past, ketamine-based techniques have been used, but not without complications such as unpleasant hallucinations.52 An i.v. propofol infusion with fentanyl or remifentanil has been advocated and provides for adequate MEP recording in 97% of neurologically intact patients, when used with a multiple pulse stimulation technique.87 MEP monitoring is less reliable in patients with a preexisting neurological decit.37 Furthermore, early hopes of greater sensitivity of MEPs over other monitoring techniques may have been premature. There are recent reports of preserved neurogenic MEPs associated with postoperative motor decit, 72 which suggest the sensitivity of MEPs is less than 100%. SSEP monitoring has become an accepted standard of care during spinal surgery. It is less affected by the technical difculties associated with MEP monitoring. MEP monitoring is, however, becoming more widely used and the two methods should be regarded as complementary, with the use of a wake-up test reserved for situations where neurophysiological monitoring is not possible or responses are signicantly perturbed during surgery and mercaptopurine.
