TA- 61. POSTERIOR REVERSIBLE ENCEPHALOPATHY SYNDROME ASSOCIATED W ITH TEMOZOLOMIDE Ivo W. Tremont-Lukats, 1 and Zoran Rumboldt 2; 1Culicchia Neurological Clinic, Marrero, LA, USA; 2Medical University of South Carolina, Charleston, SC, USA A 19-year-old man with primary diffuse meningeal gliomatosis began treatment with adjuvant temozolomide TMZ, 150 mg m 2 on days 15 every 28 days ; . One month before, he had finished craniospinal irradiation with concurrent TMZ 75 mg m 2 orally ; . On day 3 of treatment, he developed headaches, confusion, and seizures. On admission, the patient had a blood pressure of 141 105 mm Hg. He was confused and had a mini-mental state exam MMSE ; score of 20. Funduscopy, visual fields by confrontation, and visual acuity were normal. The patient had a symmetrical, intentional hand tremor. No laboratory abnormalities or evidence of infection were present. An MRI scan of the brain on admission was compared with a baseline MRI taken two days before the onset of cycle 1 with TMZ, showing bilateral subcortical and cortical lesions in parieto-occipital and posterior frontal lobes with increased apparent diffusion coefficients. We stopped treatment with TMZ and started with levetiracetam 250 mg twice daily. Three days after admission, the patient was clinically better and was discharged. We followed up 1, 3, and 8 weeks after discharge. His mental status improved but never returned to baseline. We restarted TMZ for cycle 2 at 100 mg m 2. A follow-up MRI 6 weeks after admission showed complete disappearance of the hyperintense lesions. The patient continued treatment with TMZ but had disease progression and died 7 months after admission. Posterior reversible encephalopathy syndrome PRES ; is the acute and variable presentation of headaches, delirium, seizures, and visual deficits associated with bilateral cortical and subcortical vasogenic edema predominantly in the posterior areas of the brain. The most common causes of PRES are hypertensive encephalopathy, eclampsia, and immunosuppressive drugs in transplant patients. PRES has been described in adult and pediatric cancer patients treated with CHOP cyclophosphamide, adriamycin, vincristine, prednisone ; , l-asparaginase, fludarabine, ARA-C, gemcitabine, and cisplatin, but we did not find published reports of PRES associated with TMZ in.

Case studies The unpleasant smell of hydrogen sulphide from a petroleum refinery lead to complaints and reported symptoms of headache and nausea. Data presented by Mark Alderson of Corixa Corporation showed that the mean survival time MST ; of MTb72f vaccinated guinea pigs was 57 weeks, compared to 47.5 weeks among BCG vaccinated mice and 18.5 weeks for controls. This effect was stronger when MTb72f was used to boost BCG; MTb72f-boosted BCG increased the MST to 66 weeks in guinea pigs while MST with BCG alone was 41 weeks, and 19 weeks in controls Alderson 2006 ; . A study comparing the MTb72F vaccine delivered as naked DNA to formulations of MTb72f with adjuvants AS01B and AS02A found that the formulation with MTb72f in adjuvant AS02A, although it elicited a weaker immune response, was more stable, making it the preferred formulation of the MTb72f vaccine. Guinea pigs vaccinated by either DNA or AS02A adjuvant form of MTb72f showed a survival comparable to BCG greater than one year after being exposed to infectious TB. However results from another study conducted by GSK and Aeras found that guinea pigs vaccinated with BCG boosted with MTb72f did not have an increased rate of survival Skeiky 2004; Aeras 2006 ; . In addition to this animal data, MTb72f in adjuvant AS02A was tested in twelve PPD-negative adults who did not have latent or active TB. This study showed that all three doses of the vaccine induced antibody and Tcell responses. No serious adverse effects were reported. Another phase I study in 50 PPD-negative healthy subjects administered two antigen dose levels 10 and 40 mg ; intramuscularly at 0, 1, 2 months followed by a booster at nine months. This study showed that both doses induced significant antibody response compared to antigen or adjuvant alone. There were no safety concerns from this study either. However, preliminary data from a third study in 40 PPD-positive subjects aged 18-45 showed that volunteers recently infected with TB had injection site reactions or reactions at distal site reactions. In another study with non-recently infected subjects, the safety profile was acceptable and liothyronine.
