We identified among 984 patients with unstable angina UA ; patients UA-BO ; with unfavorable short-term outcome as intra-hospital myocardial infarction ; and 18 patients UA-GO ; matched for age, gender, risk factors and clinical presentation, without any events. 20 patients with chronic stable angina CSA ; with severe coronary atherosclerosis without previous acute coronary events and 19 subjects without evidence of coronary and carotid artery atherosclerosis by angiography or ultrasound CTRL ; were also studied. CSA e CTRL were matched for age, gender and risk factors with the UA groups. Lymphotactin, Exodus-2, IP-10, I-TAC, I-309, Eotaxin, IL-8, MIP-1, MIP-3 were all strikingly increased in UA patients vs CSA and CTRL. Lymphotactin and Exodus 2 were higher in BO than GO. No differences for MCP-1, MDC, SDF 1b, GRO a, MIP-1b, MIP-3b was detected. Heat-denatured collagen is the major component of burn eschar. It should be effectively removed by either papain urea or collagenase. In this study, both enzymes showed higher levels of activity with heat-denatured versus native collagenous substrate, indicating that the burn eschar should be more rapidly removed than surrounding skin. In animal wound studies conducted in our laboratory, experimental wounds containing denatured collagenous eschar were effectively debrided by papain urea-based ointments.7 Additionally, debridement with papain urea ointments was achieved without inhibition of subsequent wound healing and dulcolax.
