The simplest method to treat small bowel bacterial overgrowth is to alter the patient's diet. If chronic low-grade symptoms are present, utilizing a high fat, low carbohydrate diet is beneficial: it reduces the food substrate which most bacteria thrive on, i.e. carbohydrates; and limits bacterial proliferation, which results in symptomatic improvement. If the patient is eating a completely oral diet, achieving a high fat intake is often not difficult; however, reducing concurrent carbohydrate intake is more of a challenge. Consultation with a dietitian is helpful in this regard and it is our experience that this diet should be rigidly adhered to on a daily basis. If the person is also receiving enteral formula, modifying the solution by the addition of fat and or switching to a higher fat formula, may make a significant difference

4322 POTT'S DISEASE IN A TEENAGER PRESENTING AS MULTIFOCAL OSTEOMYELITIS Oliva Ortiz-Alvarez, Suzanne Ramsey, Andrew Issekutz, Robert Bortolussi, Bianca Lang IWK Health Centre ; Objective To illustrate the challenge of making the diagnosis of Pott's disease in a patient with multifocal osteomyelitis. Methods We report a 17 year old Caucasian girl who presented with a 3 month history of headache, myalgias, bone pain, night sweats and weight loss. She had one day of fever of 39C. On admission to the IWK Health Centre, she looked unwell, and emaciated. Abnormal findings included bone pain, and subcutaneous nodules. Investigations showed normal WBC count and differential with increased platelet count 558 x 109 L ; , ESR 100, and CRP 185. Bacterial blood culture was negative. CXR showed lytic areas in 3 ribs. Technetium bone scan demonstrated increased uptake in multiple areas. CT MRI confirmed multiple lytic lesions of the left humerus, scapula, ribs, ileum, T6 and L2, as well as 2 splenic lesions, and 2 enlarged abdominal lymph nodes. Bone marrow biopsy showed no signs of malignancy. Bone biopsy reported necrotic bone, acute and chronic inflammation, and negative gram stain, AFB and bacterial and Mycobacterial cultures. She started indomethacin for suspected chronic recurrent multifocal osteomyelitis CRMO ; . Bone pain persisted and inflammatory indices remained high. Three months later, bone pain increased. She had soft tissue abscesses overlying the left 10th rib and T6. CNS exam was normal. Spine MRI showed swelling of intervertebral space T3- T5 ; . PPD test was strongly positive. Acid-fast bacilli were isolated from the rib abscess aspirate and culture identified Mycobacterium avium complex MAC ; . Immunological investigations confirmed an interferon gamma receptor defect. Treatment for MAC was started. Back pain, fever and night sweats resolved with no neurological sequelae. Results Obtained and Conclusion Pott's disease still represents a diagnostic challenge; a high index of suspicion is required as delay in diagnosis is common. Early PPD skin testing and biopsy for Mycobacterium should be done in patients presenting with bone lesions and a clinical picture suggestive of CRMO. If only necrotic bone is obtained on biopsy, repeated biopsy and cultures may be required. When MAC is isolated, immunodeficiency is likely, and the patient should be investigated for interferon gamma signaling defects.

Background: The present study was planned with the intention of accurately estimating Hib disease incidence in the district of Colombo, Sri Lanka. The study consisted of a prospective, population based study of Hib infections in the district. All five hospitals in the district of Colombo with a consultant paediatrician participated in the study. Children in the age group 0-5 years in the district of Colombo 179, 103 ; were the population under surveillance and the study was carried out during the calendar year 2004. Methods: Routine laboratory services were strengthened and latex agglutination test for detection of Hib antigens in CSF was used, aimed at increasing the Hib infection detection. The latex test increased the Hib meningitis detection by four fold. Results: The study documented for the first time a microbiological analysis of meningitis in infants and preschool children. All Hib infections detected were that of type b and accounted for 50% of the aetiologically confirmed meningitis. The next two common micro organisms were Group B Streptococcus 23% ; and Streptococcus pneumoniae 13% ; . Conclusions: The present study established Hib meningitis incidence of 20.1 per 100, 000 under 5 years population in the Colombo district, and estimates all Hib disease incidence to be 124.5 cases per 100, 000 under 5 years. This rate is the highest incidence reported in the south Asian region. Hib meningitis constituted 84% of the Hib diseases. Almost 80% of the Hib meningitis cases occurred in the age group below 24 months and 59% of all meningitis cases were below 12 months of age. It is known that acetylcholine ACh ; is the predominant excitatory neurotransmitter in insect central nervous system Sattelle, 1985 ; . Among insect AChRs, both -bungarotoxin-sensitive and -insensitive nAChRs have been widely studied in their pharmacological and physiological aspects Lapied et al., 1990; Benson, 1992; David and Pitman, 1993; Grolleau et al., 1996; Osborne, 1996 ; . Although the molecular structure of insect nAChRs is not as well characterized as that of their vertebrates counterparts, many recent studies have focused on characterizing the subunit composition of insect nAChRs. Although several different nAChR -type and -type subunit genes and cDNAs have been isolated from various insect species, such as the fruit fly Drosophila melanogaster Gundelfinger and Schulz, 2000; Lansdell and Millar, 2000a ; , the tobacco hornworm Manduca sexta Eastham et al., 1998 ; , the locusts Schistocerca gregaria and Locusta migratoria Marshall et al., 1990; Hermsen et al., 1998 ; , and the aphid Myzus persicae Sgard et al., 1998; Huang et al., 2000 ; , the manner in which these native insect nAChRs can be modulated by phosphorylation and or dephosphorylation induced by protein kinases and protein phosphatases remains unknown. Because of the importance of insect nAChRs as target sites for the major highly effective and widely used neonicotinoid insecticides, such as imidacloprid Yamamoto and Casida, 1999 ; , it seems necessary to better understand the intracellular messenger pathways involved in the regulation of native nAChRs. Such unknown intracellular mechanisms underlying the nAChR functional properties should undoubtedly alter the mode of action of this new class of insecticides. Consequently, in this study, we have begun to study, for the first time in insect neuronal preparations, the phosphorylation dephosphorylation mechanism involved in the regulation of the -bungarotoxininsensitive neuronal nAChRs using whole-cell, patch-clamp technique. We have found that two pharmacologically distinct types of native somatic -bungarotoxininsensitive nAChRs are differentially modulated by complex intracellular mechanisms involving PKA, an okadaic acid-sensitive protein phosphatase, and CaM kinase II. The possible cross talk between these intracellular messenger cascades is discussed. Moreover, we also report, for the first time, that phosphorylation dephosphorylation process could strongly affect the mode of action of imidacloprid, known to act as agonist at the cockroach DUM neuron nAChRs Buckingham et al., 1997 and chlordiazepoxide.

Isolation of a novel KIR2DL3-specific mAb Results ECM41 mAb specifically recognizes KIR2DL3 Mice were immunized with a polyclonal human NK cell line derived from a healthy individual donor 1 ; and after fusion, the hybridoma supernatants were first selected by cytofluorimetric analysis on the basis of their ability to stain NK cell subpopulations within the immunizing NK cell line. The reactivity of the selected mAbs was then compared by single and double fluorescence to that of various KIR-specific mAbs including: EB6b anti-KIR2DL1 and KIR2DS1 ; , GL183 antiKIR2DL2, KIR2DL3 and KIR2DS2 ; , FES172 anti-KIR2DS4 ; , Z27 anti-KIR3DL1 ; , Q66 anti-KIR3DL2 ; . By this approach we could identify a mAb, termed ECM41 IgM ; that displayed a specificity similar to that of the GL183 mAb. Indeed, as shown in Fig. 1 donor 1 ; , double fluorescence cytofluorimetric analysis indicated that the two mAbs marked precisely the same NK cell subset and that all double fluorescent cells were distributed along a diagonal, thus suggesting that the two mAbs were directed to the same surface molecule. However, the analysis of NK cells derived from a large panel of donors revealed that only in ~30% of the cases did the reactivity of the and chlorpheniramine.

This type occurs on shallow, excessively well-drained soils on S. exposures both on granitic and gabbroic soil. On gabbroic soil this type usually occurs sporadically, as small patches of less than 1 acre 0.4 ha ; . Diegan sage scrub frequently forms a border around the meadows along the W. Fork of King Creek. Dominant species include white sage Salvia apiana ; , Eriogonum fasciculatum, Yucca whipplei, Gutierrezia californica, Penstemon centranthifolius, Lupinus sp., and Clarkia sp. Mafic Southern Mixed Chaparral 37122 ; : 491 acres 199 ha ; . This is an open, low 2.5-3 ft, 0.8-0.9 m ; chaparral dominated by Eastwood manzanita Arctostaphylos glandulosa ssp. glandulosa ; and chamise Adenostoma fasciculatum ; . It occupies relatively xeric exposures on S., W., or E. aspects on gabbro soil. The two dominants are augmented with a number of other shrubs including Ceanothus greggii ssp. perplexans, holly-leaf red berry Rhamnus ilicifolia ; , Cleveland's sage Salvia clevelandii ; , Ceanothus oliganthus, and toyon Heteromeles arbutifolia ; . Prostrate or creeping sage Salvia sonomensis ; dominates in the openings and semishaded areas beneath the shrubs. Other major understory species are Calamagrostis densa, Calochortus sp. probably mostly C. dunnii ; , Allium campanulatum, Sanicula tuberosa, Lomatium dasycarpum, Pellaea mucronata, and Yucca whipplei. Six 200-m2 plots were sampled in this type. Scrub Oak Chaparral 37900 ; : 229 acres 93 ha ; . This is a relatively mesic chaparral occurring on N. aspects and slopes surrounding ravine bottoms on both gabbroic and granitic soils. It is made up of dense, tall shrubs, with average heights of 6-10 ft 2-3 m ; . In addition to scrub oak, other large shrub species such as Ceanothus leucodermis, Cercocarpus betuloides, Rhus ovata, Arctostaphylos pungens, and Ribes indecorum add to the diversity of the vegetation. Chamise and Eastwood manzanita are common and may codominate with scrub oak on W. and E. exposures on gabbro soil. Hybrid Engelmann and scrub oaks may comprise up to 10 percent cover in some areas. Several woody climbing or sprawling species such as Galium angustifolium, Clematis lasiantha, Marah macrocarpus, Toxicodendron diversilobum, and Lonicera subspicata are present. Due to the tall and dense canopy, understory cover is poor. A grayish green foliose lichen Xanthoparmelia sp. ; makes up 10-15 percent of the ground cover on steep, rocky NW.-facing slopes on gabbro soil. Species composition on gabbro is different from that on granitic soil. On gabbrro soil, scrub oak, chamise, and Eastwood manzanita codominate, all averaging about 15-20 percent cover; on granitic substrate, scrub oak dominates averaging 40 percent on two plots ; with relatively low densities of Eastwood manzanita and chamise.
