Variability. Chromosome 14 is more sensitive than expected and the sensitivity of chromosome 8 corresponds to its DNA content, however, this trend is individually variable. It appears that inter-donor variability is an important factor in the radiosensitivity of individual chromosomes and may explain some of the controversies regarding the inter-chromosomal distribution of radiation-induced aberrations. The implication of this conclusion for biological dosimetry is that the inter-donor variability is a potential source of error in calculating the dose absorbed by one individual on the basis of a calibration curve generated with lymphocytes of a different individual. A few years ago, Somerset Pharmaceuticals the manufacturer of l-deprenyl as Eldepryl ; developed a selegiline patch they call the Selegiline Transdermal System STS ; . The STS is designed to enhance the drug's efficacy and bioavailability for potential use in the treatment of depression, Alzheimer's disease, Parkinson's disease and ADHD. In May 2001, Somerset filed an NDA new drug application ; with the FDA for STS for the treatment of depression; however, in March 2002 the company received a "non-approvable" letter from the FDA requesting additional efficacy data. In February 2003 the results of a formal double-blind placebo-controlled study of the STS was published.10 The study concluded STS may have a modest, but statistically significant, antidepressant benefit compared with placebo and a similar safety profile compared with placebo in the absence of a tyramine-restricted diet. As usual, the placebos did quite well, but this study may open the door to broader selegiline use in treating depression. More recently, the National Institute of Health website reports that the STS is being studied to determine its safety and effectiveness in the treatment of HIV-associated cognitive impairment. Cognitive impairment is a common sequela of HIV infection after the onset of severe immunodeficiency.11.

In the last few decades, there have been dramatic improvements in the management of hemophilia. Hemophilia has moved from a fatal or disabling disease to a hereditary disorder and treatments that can improve clinical outcome have become available. The safety of anti-hemophilic clotting factor concentrates has been dramatically improved and no major infection transmitted by plasma-derived or recombinant products has been recorded since the late '80s. The development of virally safe factor concentrates through a combination of improved donor selection and screening, effective virucidal technologies, and the exploitation of biotechnology-engineered recombinant proteins have provided an impressive safety record with regard to pathogen transmission. Despite concerns about the potential risk of prion transmission, the major complication of treatment is currently represented by the development of inhibitory antibodies following clotting factor administration, mainly during childhood. The progressive development of long-term prophylaxis in developed countries, and surgical correction of disabilities, have markedly improved the quality of life of hemophiliacs. Current efforts are mainly focused on improving the safety of plasma-derived products and bioengineering recombinant proteins that have been modified to enhance pharmacokinetic properties and or reduce immunogenicity. In addition, several preclinical and clinical studies are currently being carried out for optimizing and individually designing therapeutic regimens using recently developed or revisited coagulation assays. Some attempts to cure hemophilia through gene therapy have been made without significant clinical efficacy in humans, but, from a clinical perspective, this represents the ultimate goal and benztropine.
Mr. A was a 24-year-old man with a 6-year history of schizophrenia, chronic undifferentiated type. He had not responded to all of the conventional antipsychotic medications. He responded favorably to clozapine, with a 35% drop in his score on the Brief Psychiatric Rating Scale BPRS ; after 12 weeks of treatment. After he had had about 8-1 0 weeks of treatment with clozapine, the nurses and therapy aides noticed that Mr. A was checking his belongings over and over again. He was also checking his bedroom door, repeatedly making sure that it was closed properly before going to sleep. His own explanation of these behaviors was, "I know it's crazy, but I can't help it. I have to do it." These symptoms did not cause any significant disruption in his daily routines, and over the next 3 weeks, the symptoms gradually subsided. Mr. B was a 34-year-old man who had a 15-year history of treatment with very high doses of various antipsychotic medications but continued to remain significantly psychotic, with various positive symptoms as well as dominant negative symptoms. Twelve weeks after he began treatment with clozapine, his BPRS score dropped by 25.
